Dosage and administration
Adults
Treatment of essential hypertension
The recommended dose is 40 mg once daily. Some patients may already benefit at a daily dose of 20 mg. In cases where the target blood pressure is not achieved, telmisartan dose can be increased to a maximum of 80 mg once daily. Alternatively, telmisartan may be used in combination with thiazide-type diuretics such as hydrochlorothiazide, which has been shown to have an addictive blood pressure lowering effect with telmisartan. When considering raising the dose, it must be borne in mind that the maximum antihypertensive effect is generally attained four- eight weeks after the start of treatment.
In patients with severe hypertension treatment with telmisartan at doses up to 160 mg alone and in combination with hydrochlorothiazide 12.5-25 mg daily was well tolerated and effective

Prevention of cardiovascular morbidity and mortality
The recommended dose is 80 mg once daily. It is not known whether doses lower than 80 mg of telmisartan are effective in preventing cardiovascular morbidity and mortality
When initiating telmisartan therapy for the prevention of cardiovascular morbidity and mortality, monitoring of blood pressure is recommended, and if appropriate adjustment of medications that lower blood pressure may be necessary.
Telmisartan (MICARDIS) may be taken with or without food

Renal Impairment
No posology adjustment is required for patients with renal impairment, including those on haemodialysis.
Telmisartan is not removed from blood by hemofiltration

Hepatic impairment
In patients with mild to moderate hepatic impairment the posology should not exceed 40 mg once daily

(Drug) has several characteristics that provide additional benefits over voriconazole, the current standard of care for invasive aspergillosis and mucormycosis. A key advantage of (drug) over voriconazole is its breadth of antifungal activity, which makes it a more versatile option, particularly before confirmation of causative pathogen. Importantly, (drug) is active against Mucorales fungi, while voriconazole is not. This offers two key benefits. Firstly, since aspergillosis and mucormycosis can be difficult to distinguish, the use of (drug) provides physicians with the confidence that their aspergillosis patients are receiving appropriate coverage when Mucorales co-infection cannot be ruled out. Secondly, overuse of voriconazole has been linked with breakthrough mucormycosis; consequently, aspergillosis treatments with activity against Mucorales may help to reduce the risk of this type of incident. Thus, (drug) is a reliable treatment choice when Aspergillus and/or Mucorales infection is suspected.

Writer, editor, researcher, and translator, Ariel N. Valerio graduated from the University of the Philippines in Diliman with a bachelor's degree in Philippines Studies. He served as editor of the UP Writers Club's "The Literary Apprentice" and the UP student publication "The Philippine Collegian" while an undergraduate student. He worked afterward with the Sentro ng Wikang Filipino -- UP System as University Researcher, writing and editing for the magazine "Daluyan" and other publications. He also served as faculty member of the Department of Filipino and Philippine Literature from 1990-2000. He worked with the UP System Information Office as managing editor of "Forum" and the AMA Computer University as Training Director. He concentrated on translation jobs starting 2005.
