Would the word "county" (as suggested by the client!) be appropriate for the Italian "provincia"?
I do prefer "province" though...
And what's more correct when traslating "la provincia di Milano/Roma/etc"?

I would say Provincia = Province as for example you say "The Autonomous Province of Trento".
However, if you have to say "Milano e la sua provincia", I would use "Milan and its surrounding area" (just beacuse English cities and towns do not have a proper administrative power, as in Italy)

It depends on the context.
I would not use "county" (= contea)because I consider this something specific to the UK/USA (i.e. with specific meaning).
In general, I would use the word "region", but if the context is a political one, "district" might be more suitable.
