contrasting translucency/opacity
A bit of a guess. Medisch Woordenboek [Mosterd/Bohn Stafleu Van Loghum] defines 'uitsparing' in a radiologicol sense as 'translucency', which, I believe, is the same as opacity. But I'm no expert by a long shot!

So in my translation I put 'filling defect' too, but I'm afraid that my limited knowledge of radiology means that I don't know how this translation might equate to the two suggestions that you kindly made, which might well be perfectly correct too!
