French equivalent for US W-2 Form

Does anyone know the word for the French equivalent of the US W-2 Form (the form that shows total hours worked, total earnings, taxes paid, etc)?

Hello.

People tend to use their last pay stub of the year for things like filling out tax forms, etc.

When you are employed by a company, there is no special year-end form that I know of.

However, when you leave a company permanently, they must send you a form showing how many hours you worked for them, how much they paid you, and that they also paid the required payroll contributions. This is called the "attestation assedic". It is proof that you worked... See more

This is called the "attestation assedic". It is proof that you worked and that your employer paid the contributions. It is used to apply for welfare benefits, not for tax purposes.
