INSTRUCTIONS: This consent form should be signed by the Female patient / Wife and Husband / Male Partner / other Family member or Representative (in this order of priority), if the patient lacks the ability to make an informed decision. The clinician / gynaecologist or his / her designee doctors are responsible for obtaining this informed consent. Patient/Partner is requested to read the form carefully and should sign it only when you completely understand the procedure to be performed and have asked all of your pertinent questions.
1. I / we, Mrs./Ms___________________________ (Female patient/wife ) and Mr. _______________________ (Husband / Male partner / Representative), , bearing Registration number ________________________, hereby give my/our consent to perform blastocyst culture (BLT) on the embryo(s) that have resulted out of In-Vitro Fertilisation (IVF) / Intracytoplasmic sperm injection (ICSI) / Intracytoplasmic Morphologically Selected Sperm Injection (IMSI) with our gametes. We understand that the clinicians and embryologists after inspection of the embryo(s) would decide whether the quality / grade of the embryo(s) are good / suitable for blastocyst culture (BLT) or not and any embryo(s) which are of poor quality or not developing normally might be discarded and blastocyst culture (BLT) is not done on their discretion.
