I can't really picture it: they have been administering Innohep in line with the patient's respective current weight at the time?

If so, would that be phrased as "Since yesterday, the administration of Innohep has been adjusted to the patient's weight"? Or is that too verbose (probably!) and a bullet point-like style is needed?
