LA DESAPARICIÓN DE LAS PARTERÍAS TRADICIONALES INDÍGENAS EN MÉXICO

Percibiendo que la transición política en nuestro país está generando una diversidad de acciones y pronunciamientos de conjuntos sociales diversos, compartimos nuestra palabra en cuanto a la lucha que el Área de Mujeres y Parteras, de la Organización de Médicos Indígenas del Estado de Chiapas, A.C. (OMIECH) está llevando a cabo desde hace 33 años. Nuestro objetivo general es: rescatar, proteger, validar y difundir la partería tradicional indígena en el estado de Chiapas.
En relación con la partería tradicional, en general, y a la partería tradicional indígena, en particular, el discurso y los hechos no se corresponden. En el discurso se reconoce, incluso se enaltece, el conocimiento y práctica social de la partera tradicional indígena; en los hechos, sus inteligencias y capacidades son discriminadas y desaparecidas por no estar “basadas en evidencias científicas”. En algunas ocasiones se les criminaliza y prohíbe su práctica al mismo tiempo que se pretende insertarlas en el sistema de salud, condicionándolas a recibir capacitaciones en las cuales predominan los contenidos biomédicos, normando así su práctica y quedando sujetas a procesos de certificación biomédica. Todo esto ha resultado, históricamente, en un progresivo desplazamiento y destrucción de las parterías tradicionales, principalmente la indígena.
Desde una perspectiva histórica podemos ver que el desplazamiento de la partería a nivel mundial ha sido el resultado de políticas impuestas por los países del Norte a los del Sur, operadas a través de proyectos respaldados por una derrama cuantiosa de recursos económicos otorgada a gobiernos y algunos conjuntos de la sociedad civil. Los objetivos, casi siempre, han sido los mismos
• El principal: acumular capital, medicalizando los procesos reproductivos femeninos, vía las empresas farmacéuticas trasnacionales que venden sus medicamentos, materiales de curación y aparatos a los hospitales de segundo nivel de atención de los países del Sur. Estos hospitales se han convertido en potenciales receptores de todas las mujeres en trabajo de parto, tengan riesgos detectados previamente o no, queriendo atenderse en esas estructuras o no.
• El secundario: reducir los índices de muerte materna, principalmente en los estados donde se ubica el mayor número de población indígena.
Para alcanzar estos objetivos han sido utilizados, principalmente, dos discursos complementarios: a) el mensaje del MIEDO para potencializar el dolor en el parto y la percepción de la posible muerte materna y/o de su hija(o), b) la fabricación de la idea de que atenderse con una partera tradicional indígena es un RIESGO. Ambas discursos tienen dos propósitos bien definidos: por un lado, crear inseguridad en las propias parteras tradicionales indígenas sobre sus saberes y prácticas y, por el otro, presionar a las mujeres embarazadas a decidirse por la atención hospitalaria y el parto por cesárea.
No es nueva la intención de borrar de este planeta las parterías tradicionales para dar paso a la partería profesional de la biomedicina. En Europa, la desaparición intencional de las matronas se agudizó en el siglo XVII. En México, a partir del siglo XIX y continuando en el XX, los frailes, jueces, políticos y médicos atacaron por varios medios a las parteras tradicionales indígenas y no indígenas. Su propósito era desacreditarlas, controlarlas y desaparecerlas para apropiarse de algunos de sus conocimientos y recursos terapéuticos: las plantas medicinales para acelerar el parto o producir anticonceptivos, las maniobras externas para posicionar a las y los bebés en la matriz, la posición vertical y la hamaca para parir, el temazcal y, lo más importante para ellos, acumular poder y dinero a través de la atención del embarazo, parto y puerperio en los hospitales públicos o privados.
Estas políticas y mecanismos de poder lograron debilitar o desaparecer algunas parterías tradicionales de los pueblos originarios en América como: Chile, Puerto Rico, Argentina, Uruguay y Cuba, entre otras. Desde la segunda mitad del siglo XX, en México, la embestida contra la partería tradicional indígena ha sido intensa. El más claro y radical caso de este etnocidio se llevó a cabo en el estado de Nuevo León, que presenta actualmente el índice más elevado de cesáreas. Incluso en el siglo XXI, desde el año 2002, pero con más estructuración desde los años 2015 a 2018, la fundación MacArthur, de Estados Unidos de Norteamérica, implantó un proyecto de apoyo ideológico, logístico y, principalmente, económico para impulsar la partería profesional biomédica en México, sentando las bases para la continuación de la erradicación de la partería tradicional indígena. Este proyecto incluyó cuatro estrategias: a) la investigación o (“línea de base”) de la situación de la salud reproductiva en México; b) la vinculación entre diversos conjuntos de la sociedad civil para conformar una agrupación nacional de parteras, cuya función principal será proponer y negociar modificaciones a las leyes y normas que impulsen la partería profesional; c) la formación/capacitación, sobre la base del modelo biomédico, de mujeres indígenas jóvenes y parteras tradicionales indígenas y no indígenas, para justificar y promover la certificación profesional y su inclusión en el sector salud nacional; d) la comunicación social para “convencer” a través del MIEDO que la única opción, válida y sin riesgos de atención al proceso reproductivo femenino en México es la partería profesional.
Por lo anterior, reafirmamos lo siguiente:
1.- La mayoría de las legislaciones nacionales y reglamentos sanitarios han sido negativos para la partería tradicional indígena, pues se han elaborado desde una perspectiva intercultural acrítica y al margen de las comunidades; es decir, de las parteras tradicionales indígenas y las mujeres que utilizan sus servicios. Con esta normatividad, desde la perspectiva del sistema de salud mexicano, las parteras indígenas tradicionales son prácticamente inoperantes para atender todo el proceso reproductivo femenino.
Ante esta situación, reafirmamos que las parteras tradicionales indígenas son portadoras de saberes y prácticas vinculadas a un territorio que defienden. Estos saberes y prácticas han sido históricamente conservados y desarrollados por las poblaciones originarias para responder a sus necesidades y fundamentar su identidad colectiva. Las parteras tradicionales indígenas sueñan, piensan y actúan de acuerdo a su propia cultura y sus derechos para coadyuvar a evitar la muerte materna.
Para lograr el reconocimiento y el respeto se deben generar e impulsar las siguientes condiciones y acciones:
• Una de las razones por las que existe una inadecuada relación entre las parteras tradicionales indígenas y el sector de salud oficial, es la falta de sensibilidad y comprensión por parte del personal biomédico. Por esa razón, proponemos que en los planes de estudio de las escuelas de medicina, enfermería y salud pública incluyan contenidos científicos provenientes de la antropología y otras ciencias sociales y biológicas sobre los saberes y prácticas de las parterías tradicionales indígenas y de su contexto. En donde sea posible, los alumnos de estos centros educativos u otros de educación media y primaria deben visitar en sus comunidades a las parteras indígenas tradicionales para conocer y sostener charlas sobre su vida y cosmovisión.
• Para una relación intercultural adecuada entre el sector salud nacional y las parteras indígenas tradicionales, los cambios en las leyes, normas y reglamentos nacionales deben ser dirigidos al personal del sector salud, no hacia las parteras tradicionales indígenas. Las parteras tradicionales indígenas de cada región debemos participar activamente para la reelaboración de estas leyes, normas y reglamentos. La Escuela Nacional de Antropología e Historia (ENAH) y el Instituto Nacional de los Pueblos Indígenas (INPI) podrían fungir como facilitadores y mediadores en este proceso.
• Los medios públicos de comunicación oficiales estatales y federales, con la participación de las parteras tradicionales indígenas, deben promover nuestro trabajo en el marco de nuestras culturas y no únicamente desde la biomedicina.
2.- Las parteras tradicionales indígenas somos parte de los recursos tradicionales que los pueblos hemos desarrollado para atender nuestras necesidades de salud y de la atención al embarazo, parto y puerperio. Por ello, nuestro compromiso y lealtad se debe a las comunidades de las cuales formamos parte y, principalmente, a sus mujeres. Si bien, tenemos necesidades económicas, convertirnos en trabajadoras asalariadas del gobierno “a bajo costo” para llevar a cabo actividades de programas públicos de salud sexual y reproductiva o de cualquier otra índole, no solo conduciría a la destrucción de nuestros conocimientos y prácticas, sino que sería sujetarnos a un control que impediría que siguiéramos brindando los importantes servicios ambientales, sociales y culturales a nuestras comunidades.
Estamos convencidas que el respeto, protección, continuidad y fortalecimiento de nuestros saberes y prácticas, como componentes de la cultura, contribuirán a la disminución de la muerte materna y a la pretendida transformación médico/sanitaria, cultural, moral y ética de México. Demandamos que
• Los apoyos propuestos por las parteras tradicionales indígenas deben concretarse en proyectos colectivos o comunitarios, para que las mismas parteras indígenas tradicionales rescaten y desarrollen sus saberes y prácticas ancestrales. Estos apoyos deben ser canalizados a través del Instituto Nacional de los Pueblos Indígenas (INPI) y no a través de la Secretaria de Salud o el Programa IMSS-PROSPERA u otro que pertenezca a estas dos últimas instituciones. El INPI deberá respetar la autonomía de las parteras tradicionales indígenas en materia de estas decisiones.
3.- Uno de los principales problemas que afectan nuestro trabajo como parteras son los condicionamientos que, la Secretaria de Salud y el programa IMSS-PROSPERA, imponen a las mujeres de nuestras comunidades para que sean atendidas en las instituciones de salud, so pena de suspender los apoyos económicos que se les otorgan. Exigimos que
• Estos condicionamientos hacia las mujeres sean suspendidos.
4.- Asimismo, a través de estos proyectos del sector salud, nos han condicionado para que modifiquemos, desplacemos o perdamos nuestros conocimientos y prácticas ancestrales de atención al embarazo, parto y puerperio; no estamos de acuerdo con que se nos obligue a coordinarnos de esta forma ni a ser obligadas a entregar informes a las instituciones de salud.
• Demandamos la eliminación del proceso de certificación.
5.- La partería tradicional indígena es el resultado de una transmisión de saberes de generación en generación. Nosotras tenemos nuestra propia capacitación que incluye saberes y prácticas que no corresponden a la forma ni al lenguaje de las capacitaciones de las instituciones de salud que nos imponen para poder expedir constancias de atención al parto. Estos cursos o capacitaciones están haciendo que nuestro saber y práctica sean desplazados y olvidados. Por lo tanto, NO estamos de acuerdo con que se nos obligue a recibir cursos sobre la base de la medicina de los doctores de bata blanca, los cuales nosotras no solicitamos.
En este sentido proponemos:
• Que se impulse el modelo pedagógico desarrollado por Micaela Icó, coordinadora del Área de Mujeres y Parteras de la OMIECH, cuya estrategia consiste en organizar encuentros entre parteras experimentadas, jóvenes y niñas, en los cuales se intercambian experiencias y prácticas de la partería tradicional indígena. Estos encuentros también serán útiles para conocer, analizar, discutir, aceptar, modificar o rechazar las propuestas de políticas, programas, proyectos, y acciones e investigaciones que el gobierno y otras instancias pretendan llevar a cabo en nuestros territorios sobre la atención del embarazo, parto, puerperio y la disminución de la muerte materna.
6.- Rechazamos que se nos culpe, abierta o veladamente, por las muertes maternas que ocurren en nuestras comunidades. Los determinantes de las muertes maternas a nivel mundial son muy complejos y diversos –la corrupción generalizada, las condiciones de pobreza, la desnutrición de las mujeres, la falta de oportunidades de acceso a una educación intercultural acordada con las culturas indígenas, la deficiencia técnica e intercultural de los servicios de salud, la inadecuada atención prenatal oficial, la difícil accesibilidad geográfica y cultural de los servicios de urgencias obstétricas, y muchos más.
Estos determinantes están enraizados en la lógica de la acumulación del dinero, a través de políticas, planes y programas sanitarios dependientes de los mandatos de los organismos internacionales: el Banco Mundial, el Banco Interamericano de Desarrollo, OMS, OPS, UNFPA, entre otros; en la explotación y apropiación externa de los recursos naturales en territorios indígenas; en el racismo hacia lo que no es europeo o norteamericano, principalmente aplicado a las culturas indígenas y, en especial, a las mujeres; en la mercantilización de las culturas indígenas y, en particular, de sus procesos naturales sanitarios; en la imposición velada de valores, entre otros.
Por lo anterior, rechazamos la idea de que es necesario transformar la partería tradicional indígena en partería profesional, para disminuir los índices de muerte materna, y solicitamos
• que las investigaciones académicas sobre la muerte materna en nuestras comunidades sean más esclarecedoras de todas las causas estructurales de estas muertes, ya que a partir de estas investigaciones también se generan ideologías, cosmovisiones y políticas de salud reproductiva a nivel nacional. L@s investigador@s y académic@s deben modificar su método de investigación en este tema específico para diseñar uno más inclusivo, relacional e imaginativo de las múltiples disciplinas biológicas y sociales.
En congruencia con los derechos indígenas consagrados en la Constitución Política de México y la construcción de un verdadero proyecto intercultural autocrítico nacional, proponemos el análisis de todas las propuestas de los grupos originarios, en el marco de un diálogo nacional, sobre la atención al proceso reproductivo de las mujeres indígenas en los sistemas de salud en México. El INPI y la ENAH podrían ser los coordinadores en este diálogo nacional.
El Área de Mujeres y Parteras de la OMIECH está integrada por 56 parteras tradicionales indígenas y aprendices, 350 mujeres socias, una promotora/encargada de la partería tradicional indígena tsotsil-tseltal, y cinco asesoras no indígenas (tres antropólogas, una psicóloga social y un médico).

THE DISAPPEARANCE OF TRADITIONAL INDIGENOUS MIDWIFERY IN MEXICO
The Organization of Indigenous Doctors of Chiapas
Our country is currently undergoing a political transition that has generated a diversity of actions and declarations on the part of different social groups. In light of this situation, we declare our position, as defined by the political struggle of the Women and Midwives’ Section of the Organization of Indigenous Doctors of Chiapas (OMIECH) that began in 1986. Our general objective is: revive, protect, validate and disseminate traditional Indigenous midwifery in the state of Chiapas.
The current discourse regarding traditional midwifery, and particularly traditional Indigenous midwifery, does not reflect reality. Official discourse recognizes and even extols the knowledge and social practice of traditional Indigenous midwives. However, their intelligences and skills are discriminated against and driven to extinction because they are not “based upon scientific evidence”. In some cases, their practice is criminalized and prohibited. Governments seek to integrate into the public health system by forcing midwives to attend training workshops in which biomedical content predominates, in this way regulating their practice and subjecting them to processes of biomedical certification. All of the above has historically resulted in a progressive displacement and destruction of traditional midwifery, namely, traditional Indigenous midwifery.
From a historical perspective we can see that the displacement of midwifery has been the result of policies imposed by countries of the global North on those of the global South. These policies are implemented through initiatives funded by a substantial overflow of economic resources directed at governments and some civil society groups. The objectives of these policies have almost always been the same:
• Primary objective: the accumulation of capital and medicalization of the female reproductive process through transnational pharmaceutical companies that sell their medicine, materials and equipment to secondary care hospitals in countries of the global South. These hospitals have become the potential receivers of all women in labor, regardless of whether their pregnancies were characterized by previously detected risks, or whether it is their wish to give birth in these hospitals.
• Secondary objective: reduce the indices of maternal death, principally in those states with the largest Indigenous populations.
Principally, two complementary discourses have been used to achieve these objectives: a) the message of FEAR to potentiate pain in labor and the perception of possible maternal and/or neonatal death, b) the fabrication of the idea that giving birth under the care of a traditional Indigenous midwife constitutes a RISK. Both discourses have two very well defined aims: On the one hand, to generate insecurity in traditional Indigenous midwives regarding their knowledge and practices, and on the other hand, to pressure pregnant women to choose hospital birth and C-sections.
The intention to wipe traditional midwifery from the face of the earth in order to make way for biomedical professional midwifery is not new. In Europe, the intentional disappearance of matrons peaked in the 17th century. In Mexico, beginning in the 19th century and throughout the 20th century, friars, judges, politicians and doctors used various means to attack traditional Indigenous and non-Indigenous midwives. Their aim was to discredit, control and eradicate them while appropriating elements of their knowledge and therapeutic resources: medicinal plants used for speeding up labor or for their contraceptive properties, external maneuvers for re-positioning babies in the uterus, the vertical birthing position and the use of hammocks in birthing, and the sweat lodge. However, their prime objective has been to accumulate power and money through the care provided to women during pregnancy, birth and post-partum in public and private hospitals.
These policies and power mechanisms managed to weaken or disappear traditional midwifery among some Indigenous peoples of America in countries such as Chile, Puerto Rico, Argentina, Uruguay and Cuba, among others. In Mexico, the onslaught against traditional Indigenous midwifery intensified in the second half of the 20th century. The clearest and most radical case of this ethnocide took place in the state of Nuevo Leon. This state currently presents the highest indices of C-sections. Now, in the 21st century, beginning in 2002 but in a more structured manner as of 2015, the MacArthur Foundation implemented a project that provided ideological, logistic and namely, financial support to promote biomedical professional midwifery in Mexico, thus laying the foundations for the continuation of the eradication of traditional Indigenous midwifery. This project included four strategies: a) research or (“baseline”) of the situation of reproductive health in Mexico; b) the linking of different civil society groups in order to form a national network of midwives whose main function would be to propose and negotiate modifications of the laws and regulations that promote professional midwifery; c) the training of young Indigenous women and Indigenous and non-Indigenous midwives using a biomedical model, in order to justify and promote professional certification and midwives’ inclusion in the national healthcare sector; d) social communication in order to "convince" the population, through FEAR, that the only valid and risk free care option for the female reproductive process in Mexico is professional midwifery.
For the aforementioned reasons, we reaffirm the following:
1.- The majority of national health laws and regulations have had a negative impact on traditional Indigenous midwifery. They have been written from an acritical intercultural perspective and without the participation of the Indigenous communities, traditional midwives and the women to whom they provide care. The perspective of the Mexican health system reflected in these regulations is one that would render traditional Indigenous midwives practically inoperative in the care provided during the different stages of the feminine reproductive process (pregnancy, birth, post-partum).
Given this situation, we reaffirm that traditional Indigenous midwives are the bearers of knowledge and practices which are linked to a territory that they defend. This knowledge and its corresponding practices have been preserved and developed historically by Indigenous peoples in response to their needs and are the basis of their collective identity. Traditional Indigenous midwives dream, think and act in accordance with their own cultures and their right to prevent maternal death.
In order to achieve recognition and respect, the following conditions and actions should be generated and promoted:
• One of the reasons underlying the unsatisfactory relationship between traditional Indigenous midwives and the public health sector is the lack of sensitivity and understanding on the part of biomedical personnel. For this reason, we propose that the curriculum of medical, nursing and public health schools draw from anthropology and other social and biological sciences in order to include scientific content regarding the knowledge and practices of traditional Indigenous midwives and their context.
• For an adequate intercultural relation between the national health sector and traditional Indigenous midwives, any changes in national laws and regulations should be directed at health sector personnel, not at the traditional Indigenous midwives. The traditional Indigenous midwives from each geographical region should participate actively in the redrafting of the laws and regulations that affect our work and knowledge. The National School of Anthropology and History (ENAH) and the National Institute of Indigenous Pueblos (INPI) could act as facilitators and mediators in this process.
• State and federal public media broadcasters, with the participation of traditional Indigenous midwives, should promote our work within the framework of our cultures and not only from a biomedical perspective.
• Renovation of the managerial staff of the Secretary of Health and the Mexican Institute of Social Security, who have been directing the traditional medicine programs for the last three presidential administrations. Their prolonged occupation of these posts is due to their ability to appear to be helping traditional medicine and midwifery. In reality, during this period the state of women's reproductive health care services in our country has continued much as it was before, or worse.
2.- We, the traditional Indigenous midwives, are a resource for our communities. Our calling and practice has been cultivated in response to our people's health needs and women's pregnancy, labor and post-partum care needs. For this reason, our commitment and loyalty is to our communities and principally, the women. While we may have economic needs, if we were to become ''low-cost'' government employees used to implement public programs on sexual and reproductive health, or any other kind of programs, not only would this lead to the destruction of our knowledge and practices, it would also mean subjecting ourselves to a form of control that would prevent us from continuing to provide important environmental, social and cultural services to our communities.
We are convinced that the respect, protection, continuity and strengthening of our knowledge and practices, as cultural components, would contribute to the decrease of maternal death, as well as to the new presidential administration's proposal for the medical/health, cultural, moral and ethical transformation of Mexico. We demand that
• Government funding designated for traditional Indigenous midwives should be provided in the form of collective or community projects so that the traditional Indigenous midwives themselves might revive and develop their ancestral knowledge and practices. These funds should be distributed by the National Institute of Indigenous Pueblos (INPI) and not by the Secretary of Health or the IMMS-PROSPERA Program or any other program administrated by these last two institutions. INPI should respect the autonomy of the traditional Indigenous midwives in their decision making regarding the allocation and use of this funding.
3.- One of the main problems that affects our work as midwives are the conditions that the Secretary of Health and the cash-conditional transfer program IMSS-PROSPERA place upon the women of our communities. These programs require them to seek care in public health facilities or risk losing the subsidies paid to them by these programs, subsidies that often represent their main cash income. We demand
• That these conditions be suspended.
4.- Likewise, the conditions placed on us through these projects of the health system have lead to the modification, displacement and loss of our ancestral knowledge and pregnancy, birth and post-partum care practices. We demand that we no longer be forced to coordinate with the health system in this way or obligated to provide health institutions with reports of our activities.
• We demand that the certification process be eliminated.
5.- Traditional Indigenous midwifery is the result of the intergenerational transmission of knowledge. We have our own form of training that includes knowledge and practices that do not correspond to the form nor the language used in the trainings implemented by state health institutions. When we do not attend these government trainings, we are denied the means for providing families with birth certificates. These courses or trainings are leading our knowledge and practice to be displaced and forgotten. Therefore, we demand that we NOT be obligated to receive courses that are based upon clinical medicinal practices, which we never requested in the first place.
Considering the above, we propose:
• The promotion of the pedagogical model developed by Micaela Icó Bautista, Coordinator of the Women and Midwives’ Section of OMIECH. This model's strategy consists of organizing meetings of experienced midwives, youth and girls in which they exchange experiences and traditional Indigenous midwifery practices. These meetings would also be useful for learning about, analyzing, debating, accepting, modifying or rejecting proposals for policies, programs, projects, actions and scholarly studies that the government and other agencies intend to carry out in our territories on the topics of pregnancy, birth, postpartum and the reduction of maternal death.
6.- We reject the blame that is placed upon us, openly or indirectly, for the maternal deaths that occur in our communities. The determinants of maternal deaths at the global level are very complex and diverse –generalized corruption, conditions of poverty, women's malnutrition, lack of access to an intercultural education in accordance with Indigenous cultures, the technical and intercultural deficiencies of public health services, insufficient public prenatal care, difficult geographical and cultural access to emergency obstetric services, and many more-.
These determinants are rooted in a logic of accumulation of capital through health policies, plans and programs dependent upon the mandates of international agencies: The World Bank, the Inter-American Development Bank, World Health Organization, Pan American Health Organization, United Nations Population Fund, among others. They are rooted in the external exploitation and appropriation of natural resources in Indigenous territories, and in racist attitudes regarding anything that does not originate in Europe or the United States. This racism is directed namely at Indigenous cultures and especially at Indigenous women. Lastly, the determinants of maternal death are rooted in the commodification of Indigenous cultures, in particular, their natural medical processes, and in the veiled imposition of values, among other things.
For the above reasons, we reject the notion that it is necessary to transform traditional Indigenous midwifery into professional midwifery in order to decrease maternal death indices. We insist
that scholarly studies on maternal death in our communities reveal all of the structural causes of these deaths, given that ideologies, cosmovisions and national reproductive health policies are generated from these studies. Researchers and scholars working on this specific subject should modify their research methods to make them more inclusive, relational and imaginative, drawing from multiple biological and social disciplines.
Consistent with the Indigenous rights embodied in the Political Constitution of Mexico and the proposal to construct a true intercultural self-critical national project, we propose that all of the proposals put forth by Indigenous groups be analyzed within the framework of a national dialogue regarding Indigenous women's reproductive healthcare in Mexico's health system. The INPI and ENAH could coordinate this dialogue.
The Women and Midwives’ Section of OMIECH is made up of 56 traditional Indigenous midwives and apprentices, in addition to 350 women, a coordinator/promoter of Tsotsil-Tseltal traditional Mayan midwifery, and 4 non-Indigenous advisers/consultants (three anthropologists and one medical doctor).

Busco un intercambio profesional con Great Cities Institute porque su enfoque en el desarrollo comunitario y económico, promueve un impacto favorable en las comunidades de inmigrantes en Chicago. Por lo tanto este intercambio sería una oportunidad para aplicar mi experiencia personal y profesional. Así como un reto para desenvolverme en un ambiente intercultural.
Durante este año de intercambio profesional me gustaría aplicar las capacidades adquiridas en mis estudios de maestría. Como se describe en mi hoja de vida, la experiencia profesional y educativa me han dado una solida formación en investigación y gestión de proyectos en el ámbito social y cultural. Como investigadora, coordinadora y facilitadora, he aplicado métodos de investigación cuantitativa, investigación acción participativa y diseñado herramientas creativas para promover el desarrollo comunitario en México y Ecuador.
Una de mis pasiones es desarrollar investigaciones que puedan mejorar las condiciones de organizaciones y comunidades, en el acceso a servicios y cultura. Me interesa participar en grupos de trabajo en donde ampliar mis conocimientos en el uso de métodos de investigación cualitativa como utilizando herramientas etnográficas, dirigiendo grupos focales, realizando entrevistas con expertos, entre otras. Además mi formación como diseñadora me permite combinar métodos creativos como design thinking y co-diseño. Al aplicar estos métodos en talleres con jóvenes he podido constatar que facilitan el desarrollo creativo, fomentan el empoderamiento y la innovación social.

En mi opinión se pueden encontrar nuevas soluciones a los problemas de la ciudad integrando las voces de quienes habitamos en ella. Considero que un punto importante para la investigación urbana es crear herramientas de diálogo entre vecinos y profesionales. Incluyendo a mujeres, niños, jóvenes y adultos mayores. Mi aporte en GCI sería generar herramientas creativas para integrar estas voces en el proceso de investigación. Mi aporte con la comunidad sería apoyar los espacios de diálogo existentes para promover el consenso entre investigadores y vecinos expertos en los problemas que su vecindario enfrenta.
Al formar parte de su equipo de trabajo, me interesa contribuiría en fortalecer el trabajo con comunidades hispanas y desarrollar herramientas metodológicas en español e ingles. Para ser difundidas que en versión impresa y digital. Así aplicaría mis conocimientos de investigadora y diseñadora para reforzar los procesos que se están desarrollando en su institución. Translation - English Based upon the questions that you sent me, I would like to explain why I am interested in participating in a professional exchange with the Great Cities Institute and the way in which my work might benefit your institute.
What would you do during the year of Exchange?
I am seeking a professional exchange with the Great Cities Institute because its community and economic development focus promotes a favorable impact in Chicago’s immigrant communities. Therefore, this exchange would provide me with the opportunity to apply my personal and professional experience, and challenge me to develop myself in an intercultural environment.
During this year of professional exchange, I intend to apply the skills that I acquired during my M.A. As I explain in my curricula vitae, through my work experience and education I have obtained solid training in research and social and cultural project management. As a researcher, coordinator and facilitator, I have applied quantitative and participatory action research methods, and I have designed creative tools to promote community development in Mexico and Ecuador.
One of my passions is the development of research that can improve the conditions of organizations and communities as regards their access to services and culture. I am interested in participating in work groups where I can expand my knowledge of qualitative research methods, such as, ethnographic tools, focus groups, interviews with experts, among other methods. Furthermore, my training as a designer allows me to combine creative methods such as design thinking and co-design. Upon applying these methods in workshops with youth, I have confirmed that they facilitate creative development and promote empowerment and social innovation.
How will this benefit GCI?
It is my opinion that new solutions can be found for urban problems by integrating the voices of the city's inhabitants. An important component of urban research is creating tools for dialogue between neighbors and professionals that includes women, children, youth and the elderly. My contribution to GCI would be to generate creative tools for integrating these voices into the research process. My contribution to the community would be to support existing spaces of dialogue in order to promote consensus among researchers and expert neighbors regarding the neighborhoods' problems.
As a member of the work team, my interest would be to strengthen the work being done with Hispanic communities and develop methodological tools in Spanish and English that would then be disseminated in print and digital form. Thus, I would apply my research and design knowledge in order to strengthen the processes under development in your institution. I would like to have the opportunity to discuss this exchange with you in order to further explicate how my ideas, experience and commitment to your project could benefit the Great City Institute.

La Organización de Médicos Indígenas del Estado de Chiapas (OMIECH) se constituyó legalmente en 1985 por promotores de salud y médicos tradicionales Tzotziles, Tzeltales y Ch'oles mayas. Nuestro objetivo es rescatar, desarrollar, sistematizar y defender a la medicina maya tradicional. El Área de Mujeres y Parteras de la OMIECH, está integrada por 51 parteras tradicionales de 4 municipios y 8 comunidades de las regiones Altos y Norte de Chiapas, México. Trabajamos por medio de talleres que refuerzan la transmisión de nuestra partería tradicional generación a generación, dentro de cada familia y comunidad. En estos talleres y los encuentros regionales del Área de Mujeres y Parteras, platicamos sobre temas como: la mortalidad materna, el cuidado en el embarazo, parto y posparto, y la conservación y defensa de las plantas y animales medicinales. En años recientes, nuestro proyecto se ha enfocado en el desarrollo de estrategias para frenar políticas públicas que amenazan la supervivencia de la partería Indígena tradicional en México. Las parteras tradicionales (en el idioma Tzotzil jtamoletik), de niña sueñan que van a ser parteras. Dios (en Tzotzil Kajvaltik, Jtotik, Ch’ul totik ta Vinajel, Ch'ulme'tik ta Ch'ul Balumil) nos da la señal por medio de nuestro sueño. Nos da nuestro material de partera. Aprendemos desde el sueño y atendiendo nuestros propios partos. También, aprendemos de otras parteras más experimentadas, nuestras abuelas, madres, y otras. Cuando una mujer nos busca, nuestro deber es aconsejarla a ella y su marido. Dependiendo de la paciente, si esta muy enferma, cansada, o si tiene mala alimentación, vamos a ver lo que necesita. Le decimos al marido cómo puede ayudar a su mujer.
Algunas parteras también somos curanderas (en Tzotzil j'iloletik “los que ven más allá”). Las curanderas tienen una comunicación directa con el espíritu. Hacen diagnóstico por medio del pulso. Este diagnostico es importante porque algunas mujeres no quieren decir que tienen problemas. Durante el embarazo, tenemos que pedir protección tres veces para nuestra paciente. La curandera sabe hacer prevención o tratar a la enferma, dependiendo si hay envidia, si le entró a su paciente un mal espíritu. En su rezo, habla tanto a la mujer como al niño, y habla con Dios. El rezo es para proteger a nuestra paciente y prevenir problemas. La partera que no es curandera también sabe rezar y pedir protección. La mujer también tiene que hablar a diosito para que salga bien su embarazo. Tiene que hablarle, entregarse, cada día. No se olvida uno.
Hasta el cuarto mes del embarazo se da cuenta si el niño viene mal. Le damos su sobada (masaje) a la mujer cada cuanto que necesite. Y así llevamos el control hasta los nueve meses. Hay veces que la mamá o la bebé es rebelde. La componemos y al rato esta atravesada otra vez. Esto tiene significado para nosotras las mujeres campesinas. Cuando está atravesada la beba es porque dejamos nuestra leña sin desatarla, o porque dejamos atravesado nuestra jalobte’. El niño copia como hacemos en nuestros trabajos. Por eso las parteras curanderas hacen tanto ‘’el secreto’’ como el rezo. Tienen formas diferentes de hacer el secreto para que se coloque el bebé.
Lo que queremos las parteras es que la mujer nos busque en buena hora. Porque es en beneficio de ellas y para el bien de su familia. Algunas parteras vamos de paraje en paraje para ver a las madres de los niños que van a nacer. Por eso es importante para las mujeres y la comunidad nuestro trabajo, para que no se mueran las señoras y también para nosotras como parteras. Hay medicina, plantas, tanto para curar como para prevenir complicaciones.
Cuando va a nacer la bebé, hay algunas mujeres que sufren mucho dolor, cuando tienen aire o frialdad. La acostamos en el petate, revisamos su estómago, lo calentamos y lo movemos y el dolor se alivia bien. Cuando ya va a salir el bebé, le ponemos una faja en su cintura de la mamá y se hinca. Si el niño está atravesado un poco, hay que poner firme su hombro, su manita, girarlo en circulo por afuera y colocarlo bien. En algunos casos se pone aceite en “su parte” de la mujer para que el niño resbale más fácilmente. Cuando nace la criatura nos sentamos con ella al lado del fuego, la bañamos con agua tibia. El cordón del ombligo se coloca encima de la mazorca de maíz y se corta con un pedacito de carrizo. Si no pasa la placenta, hay que dar a tomar a la mamá cola de tlacuache.
Después del parto se hierve un té de pimienta, clavo, y jengibre. Este té es muy bueno para que no haiga tanto sangrado y para limpiar y calentar la matriz. Además, cuando una partera sepa usar el pus, es bueno para las mujeres, para calentar la matriz y para que vuelva casi a su tamaño normal. La mamá debe de cuidarse dos o tres meses, y no debe de tomar agua fría, porque la medicina es caliente.
Las parteras conocemos a las plantas, la medicina. Es muy poderosa la santa planta. No se paró solita, sino que tiene dueña, la santa tierra. Tiene espíritu, tanto la curandera, hierbera (en Tzotzil ac'vomoletik), la partera como la planta. Cuando no conocen la planta, es que no se les ha presentado a través del sueño. Se sueña donde crece, para qué sirve. Si no se sueña como hablarle a la planta, entonces no hay comunicación. Por eso, no cualquier persona puede usar las plantas. La partera siembra y cosecha las plantas y las guarda en su farmacia casera. Pero no la corta a cualquier hora. Tiene secreto. Dicen las curanderas que si tú tienes fe, puedes ir a tomar la santa planta. Pero, si tú la niegas, quiere decir que estas burlándote de ella. Es igual que una persona la santa planta. Si tienes dos corazones, si no hay confianza, no te vas a curar. Igual es importante la confianza entre la partera y la mujer para que no haya complicaciones en su embarazo y parto.
En la OMIECH, intercambiamos saberes con otras parteras. Estas reuniones son muy importantes porque aprendemos cómo las demás parteras atienden a sus pacientes, cuales plantas usan. También son importantes para animarnos. Aunque el Sector Salud nos quiere obligar a asistir a capacitaciones donde nos prohíben de atender según nuestras tradiciones, seguimos atendiendo la mayoría de los partos en nuestras comunidades. Cuando no tomamos su curso, nos quitan nuestro apoyo y no dejan que se registre el niño. Este intento de reprimir las prácticas de las parteras tradicionales ha resultados en altos índices de cesáreas en México. Esto de las cesáreas solo beneficia a las empresas farmacéuticas que les venden a los hospitales las medicinas y aparatos para atender a las mujeres embarazadas. Mientras más mujeres vayan al hospital, más ganarán dinero las empresas farmacéuticas y se irá perdiendo y muriendo nuestra partería tradicional comunitaria. Y entonces, ¿quien va a atender a nuestras nietas? En México hay grupos de parteras tradicionales que exigen el reconocimiento y apoyo del gobierno. Nosotras no queremos que el gobierno intervenga o que nos apoye. Solo queremos que paren los ataques a nuestro conocimiento y nuestras costumbres. Por eso, vamos a seguir luchando para que el gobierno respete nuestras formas de pensar y costumbres, para que, en nuestras comunidades, no dejemos de valorar y practicar nuestra propia partería.
Translation - English Traditional Maya midwives caring for women’s health
The Organization of Indigenous Doctors of Chiapas (OMIECH) was formed in 1985 by Tzotzil, Tzeltal, and Ch’ol Maya health promoters and traditional doctors. Our objective is to revive, develop, systematize, and defend traditional Maya medicine. The Women and Midwives Section of OMIECH is made up of 51 traditional midwives from 4 municipalities and 8 communities of the Highlands and Northern regions of Chiapas, Mexico. We implement workshops that reinforce the intergenerational transmission of our traditional midwifery within each family and community. In these workshops and the Women and Midwives Section regional meetings, we discuss topics including maternal mortality; care during pregnancy, birth, and postpartum; and the conservation and defense of medicinal plants and animals. In recent years, our work has focused on developing strategies to stop the public policies that threaten the survival of traditional Indigenous midwifery in Mexico. The following is a portrait of a Tzotzil midwife narrated by the Women and Midwives Section’s coordinator, Micaela Icó Bautista.

As young girls, jtamoletik (traditional midwives) dream that they will be midwives. God (in Tzotzil, Kajvaltik, Jtotik, Ch’ul totik ta Vinajel, Ch’ulme’tik ta Ch’ul Balumil) gives us a sign through our dreams. He/She gives us the materials we will use. We learn from our dreams and by attending the births of our own children. We also learn from more experienced midwives, our grandmothers, mothers, and others. When a woman seeks us out, it is our duty to give her and her husband advice. Depending on the patient, whether she is ill, tired, or malnourished, we will see what she needs. We tell the husband how he can help his wife.
Some midwives are also j’iloletik (healers, “she/he who sees beyond”). Healers are able to communicate directly with their patient’s spirit. They diagnose their patients by feeling their pulse. This diagnosis is important, because some women do not want to tell us when they have problems. During a pregnancy, we have to ask for the protection of our patient three times. The healer knows how to prevent and treat illness that is the result of envy or an encounter with an evil spirit. In her prayer, the healer speaks to both the woman and the baby, and she speaks to God. Midwives who are not healers also know how to pray and ask for protection for their patient. The woman also has to speak with God so that all may go well in her pregnancy. She has to speak to Him/Her, deliver herself to Him/Her, every day. One cannot forget to do this.
By the fourth month of pregnancy, we can tell whether the baby is well positioned. We give the mother an abdominal massage whenever she needs it. In this way we monitor her until her pregnancy is full term. Sometimes the mother or the baby is rebellious. We correct the baby’s position and within no time it has moved again. This has meaning for us campesina women. When a baby is malpositioned it is because we set down our firewood without untying the tumpline, or because we left the batten in a crooked position on our loom. The baby copies what we do in our day-to-day tasks. This is why midwife-healers perform both “the secret” and prayer. They have different ways of performing the secret so that the baby will remain in the right position.
What we want is for women to come see us at the beginning of their pregnancy. This is for the benefit of both them and their family. Some midwives go from village to village to see the mothers who are going to give birth. This is why our work is important for women, our communities, and ourselves: we prevent maternal deaths. There are medicinal plants not only for treating and managing complications, but also for preventing them.
When some women go into labor, they suffer a lot of pain because coldness has entered their body. We lay them down on a petate (straw mat), check their abdomen, warm it and massage it, and the pain goes away. When the baby is almost crowning, the mother squats down and we put a faja (wide woven belt) around her waist. If the baby is a little malpositioned, you have to get a firm hold on his shoulder and hand, and then from the outside, turn his body in a circular motion and get him into a good position. In some cases we put vegetable oil on the birth canal so that the baby will slide out more easily. When the child is born, we sit with it next to the fire and bathe it in lukewarm water. We lay the umbilical cord on a corn cob and cut it with a small piece of reed. If the placenta does not come out, the mother should be given a tea made from the tail of a tlacuache.
After the birth, we boil a tea of pepper, cloves, and ginger. This tea is very good for decreasing postpartum bleeding and for cleaning and warming the uterus. If a midwife knows how to use the pus (sweat lodge), this is good for women. It warms their uterus and returns it almost to its original size. The mother should take care of herself for two or three months and she should not drink cold water because the medicine is hot.
We midwives know about plant medicine. The sacred plants are very powerful. They did not grow on their own. Rather, they have a guardian/provider: the sacred earth. They have spirits, just as the healer, ac’vomoletik (herbalist), and the midwife have spirits. When a person does not know the plant, it’s because she has not been introduced to it in her dreams. In dreams, we learn about where it grows and its uses. If you
do not dream about how to talk to the plant, then there is no communication. For this reason, not just anyone can use the plants. Midwives sow and harvest plants and they store them in their home pharmacies. But they do not harvest them at just anytime. There’s a secret to this. The healers say that if you have faith, you can use plant medicine. But if you deny the plant’s power, if you do not trust in the plant, it will not heal you. Likewise, it is important that a woman trust her midwife so that there are no complications during her pregnancy and birth.
In OMIECH, we exchange knowledge with other midwives. These meetings are very important because we learn about how other midwives care for their patients and what plants they use. We support each other. Despite the Secretary of Health’s attempts to force us to attend training courses in which we are prohibited from using our traditional practices, we continue to attend the majority of births in our communities. When we do not attend these courses our social welfare benefits are withheld and our patients are denied access to birth certificates. This attempt to suppress the practice of traditional midwives has resulted in high rates of cesarean sections in Mexico. These surgeries only benefit the pharmaceutical companies that supply hospitals with the medicine and equipment used for obstetric care. The more women who seek care in hospitals, the richer these companies become, while our own traditional Indigenous midwifery dies out. And if this happens, who will attend our granddaughters’ pregnancies and births? In Mexico, there are some groups of traditional midwives who demand government recognition and aid. We do not want the government’s aid or interference. We simply want the attacks on our knowledge and customs to stop, and we will fight tirelessly to ensure that our own form of midwifery continues to be valued and practiced in our communities.

I am a Spanish to English translator, specializing in the translation of scholarly texts in the social sciences and humanities. I hold a PhD in Mesoamerican Studies from the National Autonomous University of Mexico (UNAM) and am fluent in Spanish, having lived in Mexico since 2003. I grew up in Silver Spring, Maryland and English is my first language.
In 2001, I earned a B.A. in Liberal Arts (Western Classics core curriculum) at St. John's College, Santa Fe. At the age of 23, after completing a year-long research fellowship at the National Institute on Drug Abuse (NIDA) in Baltimore, I travelled to San Cristóbal de Las Casas, Chiapas, Mexico. There, I worked as a volunteer for a Maya women's theater group, began learning Spanish, and started on the path to becoming an Indigenous Studies scholar. In 2007 I was awarded an M.A. in Native American Studies by the University of California, Davis. It was during my graduate studies that I began to gain experience translating scholarly texts, mostly for my own research purposes. Upon completing my M.A., I spent a year studying Tzotzil Maya in the Chiapas Highlands' community of Zinacantán. Between 2007-2009 I completed several translation jobs (Spanish to English) for a Norwegian NGO, which also hired me as a Tzotzil Maya-English interpreter and guide. In 2018 I earned my Doctorate in Mesoamerican Studies at the UNAM (an endeavor that vastly improved my grasp of formal Spanish). My dissertation was a combination of ethnographic, political, and literary analysis regarding the work of the women's theater group that I had volunteered with in 2003 — the Foundation Strength of Maya Women (FOMMA).
From 2015-2020, I worked for the translation and editing agency American Journal Experts (AJE) as an Independent Contract Translator, translating articles and book chapters in diverse disciplines, including: anthropology, archeology, sociology, gender studies, cultural studies, geography, history, humanities, education, social work, tourism, marketing, criminology, and accounting. I also have 4 years' experience in ESL editing and am an Essay Specialist/Coach at the Mexican college coaching company Blue Ivy Coaching. Lastly, I do pro bono translations for non-profit organizations in Mexico. From 2017-2021 I worked as an adviser to the Women and Midwives Section of the Organization of Indigenous Doctors of the State of Chiapas (OMIECH), obtaining grants and carrying out projects, as part of my ongoing work as a cultural promoter. As for hobbies, it is worth mentioning that from 2008-2017 I studied Capoeira Angola (a Brazilian martial art/dance) at the Centro Esportivo de Capoeira Angola - João Pequeño de Pastinha - Mestre Pé de Chumbo (in San Cristóbal de Las Casas and Mexico City).
