Source text - English Instructions for completing the Application Form:
01. Use only Sinhala or Tamil in completing this Application form. However, column No. 02 should be filled only in English.
02. Complete this application form in clear handwriting using only Blue or Black ballpoint pen. Write only one letter in a cage. Leave a space of one cage between two words.
03. Photographs should be 1 3/8 * 7/8 colour photographs. No mark should be made on photographs to be scanned and should not be distorted. 2 photographs with the printed number of the application written on the reverse should be sent in a cover.
04. The certificate obtained from paying the due fee to the Commissioner General of Registration of Persons, the Grama Niladhari, the Divsional Secretary or any person nominated by the Commissioner General in any media specified, should be affixed. Receipt number should be mentioned. (When paid in stamps, the stamps must be pasted.)
05. If the application is attested by the Grama Niladhari No other documents are required to prove residence. If any other officer attests, documents to prove residence are required.
06. The signature or the left thumb impression of the applicant must be placed before the attesting officer.
07. The application form attested by the Grama Niladhari should be countersigned only by the Divisional Secretary / Assistant Divisional Secretary or Administrative Officer.
08. When completing numbered columns, the following instructions relevant to the numbers should be followed.
1. Full name as appearing in the Birth Certificate / Approximate Age certificate/ citizens certificate / Dual citizenship certificate / Certificate confirming priesthood / Adoptation certificate should be filled according to the following formal. If there is no family name or surname the relevant cage should be kept blank. (citizens certificate) means the certificate of obtaining citizenship upon registration under Citizens Act No. 18 of 1948 or the certificate of registration of persons of Indian Origin who had obtained citizenship under relevant provisions for that purpose or the certificate obtained by persons of Chinese origin upon registration under Act No. 38 of 2008.
